The Madonna is revealed during a Mass, which is always absolutely packed to the rafters. But on one of the monk/priest led tours, you just bypass the Mass and crowd, and walk past the icon. I can’t imagine a requirement to do so on your knees - that would take until the next day for the crowd to pass by. It is possible that some of the more devout pilgrims may complete the last part of their journey on their knees, but in the church itself, that would be almost impossible.
